Form V - to change the spelling or to add Maltese fonts and Maltese accents (à è ì ò ù À È Ì Ò Ù) to the ‘name to be called’ on an Act of Birth – Article 278A of the Civil Code (Cap 16 of the Laws of Malta).
Form TT – for applicants born in wedlock, in Malta before the 29th December 2020 – Article 4(8) of the Civil Code (Cap 16 of the Laws of Malta).
Form UU – for the spouse or children of an applicant who made use of Form TT – Article 4(9) of the Civil Code (Cap 16 of the Laws of Malta).
Form WW – for persons born out of wedlock, in Malta before the 29th December 2020 – Article 92(6) of the Civil Code (Cap 16 of the Laws of Malta).
Form XX – for the spouse or children of an applicant who made use of Form WW – Article 92(9) of the Civil Code (Cap 16 of the Laws of Malta).
Form Y – for spouses married in Malta before the 29th December 2020 – Article 4(6) of the Civil Code (Cap 16 of the Laws of Malta).
